If I may give a suggestions.. (not sure if you can still make alterations to the survey)

This question:
When is it ok for your children to eat unhealthy snacks (besides special occasions)?

I feel that the range of choices is not wide enough. You could add "once a month" or even "never"

I believe that balance is the key. The more children are told "no sweets" the more they crave that which is forbidden. My children were allowed something sweet once a week. Sort of a reward system. If they had completed all homework assignments and chores for the week they could have the "unhealthy" snack of their choice.
